Sex Differences in Renal Outcomes After Medical Treatment for Bilateral Primary Aldosteronism

A higher incidence of bilateral primary aldosteronism in women is reported. Treatment usually involves mineralocorticoid receptor antagonists. However, the impact sex on renal outcomes unknown. We compared between sexes after antagonist initiation by analyzing data obtained from 415 female and 313 male patients with who were treated spironolactone or eplerenone JPAS (Japan Primary Aldosteronism Study). Over course 5 years, temporal reduction estimated glomerular filtration rate was greater than men ( P <0.001). Systolic blood pressure levels equal sexes, despite doses antihypertensive drugs men. The mean annual decline during what we termed late phase, 6 to 60 months initiation, larger adjusting for patient characteristics (−1.33 mL/min per 1.73 m 2 year versus −1.04 year, <0.01). Female a significant predictor phase taking but not those eplerenone. Spironolactone use diabetes independent predictors women. These findings suggest that associated poorer receiving aldosteronism.
